Latest Patch - massive VATS problems

Not sure if this is entirely specific to the lever-action shotgun, but ever since the very latest patch (the one that fixed the invincible enemies thing) both reloading and VATS are totally jank.

When reloading, if I'm holding down right-click while it's going on (to try to get back into iron sights as soon as it finishes), lord only knows what will happen. Sometimes it works, sometimes my gun is stuck in a weird elevated position, and sometimes it lets me go into iron sights normally but won't let me actually get out of iron sights for a few seconds. Maybe an even split between all of those, it feels totally busted.

But the big problem is trying to use VATS. I'll paint the scenario I'm in right now in case it is more specific than I think:

Boulder City. I steal the lever-action shotgun from the back room of the bar. Get the quest to deal with the Great Khans there. Free the NCR troopers, kill the Great Khans that are outside. Head into the hideout to confront Jessup.

This is where it gets weird. If I enter VATS once I get inside this room, it works fine as far as allowing me to choose my target and queue up a round of attacks. When I try to launch those attacks though, the camera goes into cinematic mode, showing my character without any animation and he starts sliding in a random direction. The first time I started sliding in circles around Jessup, not firing any of the attacks I queued while him and his friend wailed on me in VATS. This persisted until they killed my character during the VATS sequence.

The second time I tried this, I killed his pal just using iron sights, then queued up a round of VATS on his face. When I tried to initiate that round, my character slid backwards into the back of the room (where the rubble is), while Jessup slo-mo chased him trying to get him back into line of sight. After about seven or eight seconds of sliding into the wall without animation, my character finally launched all of the attacks I had queued up.

I currently don't have any mods installed or anything like that. Never experienced this before, and I've used this gun plenty in my last playthrough.
