Delete your game data and the patch, and then reinstall both. Do not delete saved games. Reload your last saved, you should be good for now, at least with this problem.

Got same thing happening all over...
Re-installing gamedata doesn't work...
Sometimes it does but then 10mins later [censored] happens again!...

Glad it worked. Check the bottom of your disc and compare it with another game to make sure there are no abnormalities.
