Are all horses essentially the same horse? Can I spend a bit more money on one a bit tougher/faster/not so obese?

There are a least two special ones, one should be faster, one should be stealthier (both have quests attached). If they are or not I couldn't say.

No, the horses in skyrim are more stocky so they are more durable. They trade in speed for durability.

Except a super horse you get from the dark brotherhood, I think it's the same speed though, It just has more HP

You get stockier animals in colder climates, I can't imagine an Arabian would be happy running through the Norwegian winters.
