Do you have to play through the main quest a little bit...

For dragons to spawn anywhere? In other words, do you have to play through the main quest to get up to the point where you fight the first dragon and become the dragonborn for dragons to start to spawn randomly?

Yes. I put in 10 hours without doing any MQ and I didn't see a single dragon.

If you don't do anything with the Main Quest, no dragons. Although some side missions require you to be known as Dragonborn, so you'll need to do some.

I believe so as I didn't see any dragons until after I completed this quest.
