I'm constantly being surprised by all the small details they added to Skyrim, like the butterflies, moths, bees, birds...it's just amazing. I almost drowned while trying to get close to salmon that were jumping out of the water.
One small game mechanic that I appreciate a LOT is the tiny text that says [empty] when you're checking the contents of containers. Also, flowers actually disappear when you pick them. Whew! Now I'm not wasting a million clicks, looking into the same darn barrel to see if I already took the ball of yarn out of it.
Some of the subtle sound effects are amazing too. In the first dungeon I explored, there was an area of connecting caverns. There was a faint howling sound in the area, like wind, but once in awhile it would start to sound like voices muttering - and I would spin around, expecting to see something or someone behind me.
