How come sometimes when I kill a bandit car i'll get the Proof of Death to turn in to Sally for rewards and other times I don't? I've had it happen during other missions so thats not it.

Yeah, I've reported this. In the Scorcher territory, near the entrance to Dead City, they don't give kill confirmation skulls.
