Companion and House bug

Companion equip item bug:
So far I've only seen this issue with armor. A recurring bug is that even though my companion is equipped with "EPIC" steel plate armor (that I smithed at the forge); if I give him any default steel plate armor (briastplate, gauntlets, etc) to store from my inventory, he will automatically equip the 'default' weaker armor. I have to take the 'default' armor out of his inventory for him to equip the superior armor that he was already wearing. So far this only appears to occur if it's the same 'type' of armor. Anyone else experiencing the same issue?

Companion AI bug?
1. In one of the dungeons (a cavern filled with bandit archers) where there is a large pool of water (I think the one with the bandit leader in the cage/arena area). Two bandits fell in the deep water and were just swimming around. My companion goes in the water after them and then they just stare at each other face to face in the water while treading water....this goes on for 20-30 minutes until I was finally able to kill the two bandits in water (below the surface, which is basically shooting blind) with my arrows. Only then did my companion disengage and follow me.

2. Companions refuse to walk/step over flat irregular raised surfaces that my character can easily just walk/hop over.

Housing bug:
When I 'walked' into my house in Whiterun, all the small items (plates, fruits, etc) spawned in the air and fell to the floor. These were the default items that came with the house and it only affected the main floor. I reloaded the last save and nothing reset and was still sprawled out all over the place. I think this is a physics engine issue. I don't dare waste my time putting everything back in place as I found out that anything I place, that was not included in my house upgrades, will just end up on the floor the minute I walk back in the house. Thus bug did happen immediately. Everything worked fine and stayed in place for at least a week worth of playing.
