Ah. I saw this in a live stream earlier this week. I thought it was odd, too. Have you checked to see if you've contracted any diseases?

Its a marketing trick. They say you look sickly, and hungry. Then they offer you some food. You ask for more food, and they raise the prices.
Trust me...you're...you're fine!

Might be vampirism. I got it on the first vampire I encountered. I even left it a few days and I could still cure it via praying to Talos.

I think it's probably this isn't it?

I've had the same thing happen to me. The most annoying part is that the game doesn't tell you that you have a disease, never mind which disease you have. It doesn't tell you what skills/attributes the disease effects. It doesn't tell you anything!! They should have kept this as it was in oblivion. I drank a cure disease potion but I aint got a clue if it worked. Am I missing something????

It does work the same as oblivion. You might just be ugly.

Spoiler
Though the alchemist in Whiterun will tell even healthy people they look sick. She's probably just trying to sell you something.

I wondered that too. Turned out i did have a disease. Look into Magic->Active Effects.

Also praying in a temple cures diseases.

It will tell you on one of the menu screens which disease you have and the effects. I caught one and had everyone telling me I looked sick. Probably got it from a wolf bite? One potion and it was gone.

Dammit!! Can't believe I missed that. Don’t use magic much so don't go into the magic menu very often

Thanks very much for the help guys!

I figured this out. It's because you haven't been sleeping or eating. Eat food at least once a day and sleep 5 or 6 hours.
