Max volume too soft (not the 4496 hz problem)

The problem I have is max volume is a bit soft (and way softer than any other game I play). There are threads on steam and gamefaq forums with other people who have this issue so I do not think it is a one off but I havent' found a fix that works:

hardware/os: msi p67 (sandy bridge) with on board realtek audio chip using windows 7 64x and ati 6950.
I have checked mixer (the msi driver does not have an option for the mixer)
I have tried modifying skrimperf.ini by setting max volume up to 4.0 (didn't seem to do much).
the frequency is set to 44k (i realize there is a problem with folks who set it to 96k get static or crashes)

I suspect from other threads this might be related to the realtek but not sure why skrim would be the only game with this issue.

Anyways is there a known fix or any chance of a patch to fix this issue ?

unrelated but small comment would be nice if the esc key worked like the tab for getting out of commands.

Tearn on SVM / Loundess Equlisation - That fixes the range of noise BUT if your unlucky like me, things like fires and the physics issues with things wobbling on plates indoors will deafen you:
