If Item X is fixed from y bug, fine. Say so. But to say that it's "rare" or that it's "infrequent" is editorializing, and under the current state of bug fixes, can be a bit on the insulting side to the reader.
List the fixes. Keep the editorializing to internal memos that never see the light of day.
