I've seen mods like Tales of Lycanthropy, which allows you to infect companions with Sanies Lupinus, which makes companions transform, if they are infected, when the player chooses to transform. So this is definitely possible, although it doesn't sound very simple but it would be amazing to have Serana transform into a vampire lord when the player does as well.
