or can I play the game on any difficulty to get them?

I don't know about Ghost. I can't unlock that achievement no matter what I do so I can't speak to that.

I have got both Clean Hands and Shadow playing Normal difficulty. Difficulty usually only factors in when an achievement is specific about it. Like "Completed Game on Insane Difficulty" or something.

i completed the entire game on normal in one go without any kills or detections, still i wasnt awarded with ghost and clean hands...

I've completed the game without any kills either but still no "clean hands" achievement. I'm one stage short of "ghost" though. I wonder why these achievements aren't unlocking, I finished the final mission twice.

There are some glitches that invalidate Clean Hands. For example, if you choke out / sleep dart Granny Rags
in Flooded District, the game considers it a kill, but this is never listed on the mission stats screen.

I really hope they fix that.
