So in the high chaos ending

Corvo dies? All it shows is his grave and says "In loving memory"

Of old age yes. As you can see Emily probably around 40 is standing over his grave. Only ending he doesn't die in, is the True High Chaos ending.

That's the one you get if you kill all NPCs, end every mission in High Chaos AND let Emily fall.... right?

Yes that is the one. Main requirements for it are killing Perio, Sokolov, and Emily.
