Since installing Hearthfire the game does not remember my Gameplay/Visual settings.

I play on Master difficulty, but every time I load Skyrim up, it resets to Adept in the settings.

The same goes for the brightness level and subtitles options - they get reset too.

I've noticed my game changing from adept to master and vice versa. I never go into the settings, so i know for a fact i didn't change it. I only checked when i went from 2 shotting some enemies to 5 shotting them. Add to that, i was taking more damage.
