I am wondering if poisoning the Bottle Street Gang's knock-off elixir will affect anything in the ending or the game? If so, can you please explain what and how does it affect it?

By poising the bootleg elixir the Bottle Street Gang is selling and using themselves. You create more weepers and rats seen in the later levels.

I don't think that adds to your chaos level, as you are not killing anyone. I'm not entirely sure. And the amount of Chaos changes the ending you get.

I think Chaos increases when you:

• Kill a lot of people
  
• Get spotted
  
• Attack innocent people
  
• Go in guns blazing rather than Stealthy.

Might be more things that increase it.

Will this impact the achievement clean hands?

No. It doesn't count as killing anyone, and it doesn't raise your Chaos rating, and it does not affect the ending of the game. As someone else said, it only effects whether or not weepers are are present when you return to the Distillery on a later mission.

Untrue if you complete the poisoning the elixir sidequest and don't kill anyone the entire game you will still not get the Clean Hands achievement. The guide even says to avoid this sidequest if you want to complete a non-lethal playthrough.

Eh, I didn't realize it affected the achievement. Which is weird, because it definitely doesn't affect Chaos rating, the ending, or the kill count for that stage.

I just finished the game and I can confirm that poisoning the elixir DOES NOT void the Clean Hands achievement/trophy.
