the heart you carry around is quite clearly hers, it has also quite clearly had some mechanical alterations to it. and in one of pieros audio logs he talks about possibly being able to preserve a soul, by keeping the heart beating. It seems like thats pretty much what has been done to the heart. Someone did something to it to keep it beating. Piero isn't anywhere nearby to do it, though if piero could do it it would be assumed that sokolov would as well. though he doesn't really have any motive to.

also while the outsider does give you the heart it doesn't mean he had any part of its creation.

Sounds plausible. I believe maybe Peiro did make the heart, because he got the idea to make Corvo's mask from a vision the Outsider gave him. The heart also said that the Outsider vitis Peiro every night in his dreams. It could be the same for Sokolov but the Outsider himself said he does not find Sokolov interesting.

The Outsider clearly states that he crafted the heart himself. Piero's musings was just a way of implanting the idea that the heart contained the Empress' soul. Obviously, he got the idea from a dream from the Outsider, but I doubt he'd have the knowledge in magic to do something like that.

Oh yeah I forgot about that. After reading so many books and listening to so many audio logs some information just gets pushed out. lol

EDIT: just played through the part where he gives you the heart again. he says "...molded by my hands" doesn't necessarily mean he created it. He could of taken it and done something to it so that it would locate bone charms and runes

Never said that the Heart itself, was magic. I'm just thinking that some sort of magic was probably involved in it's creation due to it's nature. Also, in the audiograph, Piero states that he's reluctant to pursue avenues that have come to him from his dreams.

I checked when you 1st get to the pub for the 1st time, and when you get there after the high overseer mission. didnt think to check after the dream

Clearly there's magic involved in the heart's making, since right-clicking it makes Jessamine talk to you as a ghost and give clues about the area that you're in. My immediate guess is that Piero made the heart itself and the Outsider infused it with Jessamine's soul and the ability to detect runes and charms.

@malavok keeping the heart beating keeps the soul from leaving, so there is no need for the outsider to infuse it with her soul.

also I was just playing through again, and I thought of a likely scenario, based on both pierros audiolog, and what the outsider says to you. it seems like the outsider came to pierro in his sleep gave him the idea to do it, and how to do it. and since it is the outsider who did this it is possible that the ideas he gave pierro also had some sort of magical effect, or maybe even just the act of keeping the heart beating which in turn essentially traps the soul in the heart has supernatural side effects which makes the person holding the heart hear the soul, and react to bone charm/rune. and that what the outsider did to/with pierro is what he was referring to when he said he molded it.
