Nope even if you get to stage 4 you will no longer be attacked. It will still say as a blood-starved vampire you are hated and feared but you wont be. It was something that was put forward in the Beta, back to your original question no you dont have anything to worry about.

You can still go to town. Nobody will be hostile.

Just say you are a Skooma user. Nobody will pay you a blind bit of notice.

normal in humanoid form...vamplord u dont notice the difference due to svcking the life out of what just hit u...magicka regens in sunlight but not stamina...dunno if its meant to be that way

Yes in dawnguard stage 4 no longer are attacked. Now if you switch to Vampire lord form your on your own
The more stages you progress the more magicka, health, stamina, it will take away and at stage 4 you will not regenerate any of those 3 until you feed again
