Okay. I'm a little frustrated. I've played through this game, and prior to attempting the clean hands run... read up on everything that might screw me over.

I avoided granny rags completely so as to not deal with that mess. I killed absolutely no one, alerted no one. In mission 3, I used sleep darts on the weepers in the sewer (and took care to make sure they did not fall into the water).

I just finished my supposed clean hands run. No achievement. All my missions say 0 kills, never detected. What can I possibly be missing? This is extremely frustrating D:

edit: I also did not kill the assassin's in the prologue, nor did I kill any hounds. I do have a comprehensive save history.. so if anyone has any bright ideas?
