I knew about censorship in Japan and India, Fry's dog, Ford Nucleon and A Boy And His Dog. So that's 5 out of 10.
I'm not sure I believe the bonus question. I'd never heard of that before, and a quick internet search only turned up references to the Fallout games. But if we do count the bonus question then it's 5 out of 11.
Better than I! I, too, had heard about the "tweaks" for the japanese release and understood why, but not the one for India. That's a whopping 1 of 10 (or 11, counting the bonus)
Surgery Mini-game I didn't know about, glad they didn't go with that. I am also happy Bethesda didn't change Brahmin to make India happy.
Also if I remember right the bonus thing is made up stuff.
The answer they gave to the bonus question is incorrect. One of the original developers (maybe Tim Cain) said that the reason the Vault Boy has his thumb up is that he is just a positive kind of guy (I'll see if I can find a link to the comment).
EDIT: This isn't the comment I remember, but Tramell Ray Isaac, an artist on Fallout and on Fallout 2, https://twitter.com/Dirty_Camaro/status/529308243404017664, "it's just a thumbs up, basically saying everything is ok, when it really isn't ..nothing more than that."
