first one: Lily companion bug, i sent her to do the experiment with the doctor in jacobs town, i talked with the doctor before she arrived, not knowing i could start it without her, now she is MIA

second one: After sending ED-E back to the lucky 38 i fast traveled to the strip entrence, the gate had been locked and the gaurds said nothing, yet to be resolved

third one: Due to the second glitch, i thought i would just have to progress with the story, so i decide to go one with the Caesar mission, once there the guard outside his tent tells me i have to go in alone though i have no companions with me, i have noticed some people mentioning ED-E with me however

For your ceaser mission go find ED E and tell him to go back to primm try that

i would but he's no where to be found, and i fixxed them all except the ED-E bit, at the cost of having to turn hardcoe mode off and killing the caesers legion and the guard bots outside the strip, costing me a trophy and forcing me to kill my favorite faction
