If the timing is fixed, that is almost certainly some sort of background service causing interference. If the timing varies, depending on what you and your character are doing, you have five elements to deal with regarding graphics images, and if it's not tied to motion, we can eliminate one of those. You have tested with drivers and a fresh copy of Direct3D, which leaves only the game itself to uninstall and reinstall (the fifth, motion- involved element is PhysX), or something is physically wrong with the graphics card.
