I am a level 21 right now and have a pretty balanced character build.My melee and medicine are low though.What areas should I bulk up first before I jump into Dead Money?

Lol your melee. But you should be ok.

Melee, Unarmed, and maybe Speech, but there are a lot of dialouge checks that are based on skills other than Speech.
