Two constant V.A.T.S. bugs

Am I the only one who has had these problems, or is it just something that keeps getting ignored in patches?

The first one I've noticed since Fallout 3, and it reappeared in NV and has yet to be fixed. Occassionally, when queuing up a few shots in a row in V.A.T.S., with no obstacles in the way and even at 95% chance to hit, the first shot will hit and then the camera will go off to something irrelevant and the rest of the shots will be heard rapid-fire but not hit anything. This can happen when engaging a single target, or queuing up against multiple targets. I've occassionally noticed weapon effects being applied when this happens (holorifle damage for example), but without the damage that would be appropriate for a hit.

The second one seems to be new to NV and has been a constant disappointment for me, considering the Western atmosphere of the game. It seems that all revolvers are bugged and will only occassionally land shots in VATS even with high chances. I've noticed it the most with headshots, where only one out of every 3-4 shots will land even at 95%. I've occassionally spotted the tracers flying overhead, so if I had to guess I would say that it looks like the guns are somehow shooting over the heads of the targets when being told to shoot at their heads. At any rate, I've noticed this one with everything from the .357 revolver to the Ranger Sequoia, and most recently the Police Pistol too. I've wanted to make a "Cowboy" style character since the day NV launched and I still can't do it because revolvers are almost useless due to this bug.
