I don't know how, as it's supposed to be impossible, but while throwing a hatchet at the large rat in Freeside, I hit one of the children who was chasing it. This was done in VATS and I got a nice slow motion view of it. I know the game's engine actually registered it because a message appeared in the top left corner saying "Child torso crippled."

Needless to say I am disgusted bethesda could let something so profane slip into their...hahahah just kidding
