Its.. Batman.

but seriously, Its a more realistic mode, Which makes you need the following: Water, Sleep, And food.

It does'nt increase difficulty, It makes ammo have weight, And companions can die.
Hydra, Doctors Bags, are needed to heal limbs, Rad-Away and Stimpacks restore/remove HP/radiation overtime.

Basically, you need to drink, eat, sleep, ammo has weight. Stimpacks heal over time. Only doctors, doctor's bags and hydra heals your limbs, Companions die. There might be some other things I forgot but that's gist of it. It's not really that bad, I played it straight away, early game it's a little bit of a struggle but as game goes on you get used to it and it almost seems normal.

Adds the need to eat, sleep and drink as well as ammo weight. Makes it so you can't heal crippled limbs with stimpaks and stimpaks take longer to heal.

You need to drink or you die.
You need to eat or you die.
You need to take a nap from time to time or you die.
Stimpacks heal over time and not instantly.
Companions die and don't get knocked out during combat.
You cannot heal limbs without a doctors bag, hydra or at a doctor.
Ammo has weight.

Companions don't die unless you use hardcoe mode? That's stupid.

It's basically more realistic, in some areas of the game. You have starvation levels, dehydration levels, stimpaks heal over time and not immediately, they also don't heal crippled limbs. Ammunition has weight, companions can die, rad-away heals radiation over time not immediately. Things like that.

You can still play on easy difficulty, hardcoe just means you have to be more aware you know. Decisions should be based on more realistic outcomes. It's mostly there for the challenge.

One of the series tenets ~NPC's die with no recourse; Be careful with your NPC's.
