Your game must be broken then.
I never had a companion die either, they will go down but they always get back up when I've defeated the remaining enemies.

The ones that just got lost make me the most sad. No clue where they went.

Rest in Peace, Faendal.

It took me awhile to even notice he was dead. Eventually I returned to the last place I saw him, and found his crushed body under the skeleton of a Dragon.

I have gotten most of the companions killed one way or the other, but most of them show back up later on except Lydia. The thing I can't figure out is after the last one got killed. whenever I try to get someone to come along the reply I always get is "it looks like you have someone already" Problem is I havn't seen that guy in 30 hours of game play lol. Guess I'm a loner now.

I always feel guilty if a companion dies on me, especially if they accidentally got in the way while i was taking down a bad guy, so anytime they do i reload and make sure they are ok

R.I.P. Brelyna from Winterhold College -> Died because she was caught in magic crossfire when assassins came for me in the Hall of Elements.

R.I.P. Faendal -> Killed him because I thought he was having an affair with my wife Camilla.

R.I.P. Sven of Riverwood -> Brought him into Embershard mine where I killed him, ate his flesh, then tossed his body into an underground lake.

Hmmmm, I thought I had lost more.

I am pretty sure I killed Lydia once when I was a High elf, but I am not certain, all I can remember was that she died really quick after I got her and I didn't care much when she did.

RIP Lydia
Went down from a Giant AND a mammoth attacking her, and at the exact time she went down, a dragon breathed frost all over her, killing her and the Giant instantly.
I was so angry i plummled the dragon in mere seconds of rage, and quickly dispatched the hunk of meat and tusks.

maybe it depends on the difficulty setting. i think companions only die if you kill them. maybe i'm wrong?

wait here we go :

Follower Death

The game treats permanent (non-canon) followers in a similar way to "essential" NPCs, but still allowing the possibility of their death. When your follower would die to an enemy attack or a trap, they will be incapacitated for some time and begin to recover their health, just like essential NPCs; enemies will stop attacking or targeting them during this time. However, if they lose all their health again during this period, they will die for good. Thus, you should try to move the fight away from them, so that they are not hit by a stray arrow or area-effect spell. They can also die if hit with a damage-over-time poison that is stronger than their health recovery. They can also die from enemy power-attacks.
If you strike your follower dead, on the other hand, there is no recovery. Attack carefully when your follower is low on health. Follower death by yourself, even accidentally, counts as murder on your crimes list.
Many temporary followers are "essential", so they cannot be killed, only incapacitated for a while. Non-essential ones will die immediately when struck down, just as ordinary NPCs.
To ensure follower healing, make sure you give them healing potions and food to carry in their inventory. This way, once a follower is "wounded" they may heal almost immediately and return to the battle. If a follower isn't carry any healing items, you may have to wait until all enemies in the area are removed before they will heal themselves.
[edit]

so like i said...followers can only be killed by you lol. Well only you can get them killed. So protect them from being killed by others.

RIP Faendal or whatever your name was. If I hadn't used that shout on that mountain pass to kill those hired thugs....he was already kneeling over in pain. then FUS RO DAH! Poor fellow. killed all four of them. it was a necessary death. me and the dog got away at least

RIP Lydia. Never figured out what happened to her... we were in a cave, I think she might've fallen into the water and drowned.
RIP Dark Elf chick Jenassa... never liked her too much, but felt bad after rotten Spriggans pounced us--I ran, she didn't.
Erik the Unready... er, Slayer ... ditched him after the first couple dungeons, didn't want to answer to his father for his death, only a matter of time with that one.
I like Stenvar, kinda reminds me of Jayne from Firefly; and Belrand (Hulk Hogan), except he keeps summoning his wolf right in front and blinding me.
