About certain settings in the game.

Its been a while and i have forgotten what to do about Vsync and other setting changes

I know turning it off is pointless, but i remember advice on using a program or something third party to smoothing out the game or something, but i really dont remember.

Could anyone enlighten me?

I think you need a third party utility to turn off vsync. Was it... Nvidia inspector...? Riva tuner...? I usually turn vsync off in games but I don't know... everything I have read on vsync and Skyrim has led me to believe that it isn't worthwhile. Update your drivers and fire up the game. See how it runs.

Don't disable VSync in Skyrim. Game physics and script timing is tied to this setting being left alone. If you disable VSync, you will experience some very odd and possibly game-breaking behavior. If you are concerned about input lag, play the game in a window or use a frame rate limiter. There is a "simple borderless window" mod on the Nexus that allows the game to play in a window but appear full-screen. If you have an Nvidia card, update it to the latest drivers and use Nvidia Inspector to limit the frame rate to the refresh rate of your display.

Prior to 1.3.10, the executable was not optimized so many people used TESVAL which was then replaced by Skyboost. The game has since been updated to include an optimized executable so these tools are no longer necessary. The only other thing I would suggest is that you use the Unofficial Skyrim patch, as this fixes hundreds of issues. This alone makes Skyrim worth trying again.
