Every time I assign a hot key and related L/R choices. They seem to stick...sometimes. For example I'll assign hotkey #1 to Flames in Left Hand and Frost in the Right hand.

Nine times out of ten after some amount of time (or during the next fight) I'll have flames assigned to both right and left hand.

or...

I have a healing hotkey. Healing myself to LH and Healing hands (others) to my RH. Virtually, every time I hit that hotkey, Healing is assigned to both RH and LH slots.

It doesn't matter if I save immediately after assigning them.

Is there a way to lock in your choices in the favorites menu that I'm missing? This part of the game honestly svcks. And I absolutely love this game.

from what i see i "saves" ur last positions of the spells/weapons. Also ive noticed if u are using 2handers and go to a spell it automatically goes duelweilding that spell. i dotn use a controller so i dotn know how to fix ur problem, im still trying to figure out if the keyboard and mouse have any "hotkeys".

I hear what you're saying. I wish each hotkey had an option for each hand.

1 - RH - Sword LH - Shield
2 - RH - Sword LH - Spell
3 - RH - Spell LH - Dagger
4 - RH - Axe LH - Empty
5 - RH - Bow LH - n/a

etc

I'm on PC and I haven't been able to hot key anything. I can "favorite" watever I want, but the hot key function doesn't seem to be there and there's no documentation regarding HOW to hot key that I'm aware of. Normally, you would highlight the item you want to hot key and then press the number on the top of the keyboard, but that does nothing, and I've also tried to use different methods, but nothing has worked so far.

you open the favourites menu, hover the mouse over the item you want, and press the number you want to hotkey it to.

no probs, that one had me stuck at first too.

What happens is that one-hand weapons go into your right hand, spells go into your left, regardless of your setup...bows on the other hand work a bit differently, because if select a bow while you are equipped with other stuff, when you deselect the bow by hitting its hotkey, then the original stuff returns to your hands.

If you double tap a spell hotkey, it equips in both hands. but if you have (for instance) a healing spell in your left, and you hit another spell hotkey, that second one will also go into your left hand, unless you double tap and then it goes into both.

It would be nice to be able to allocate spells/equipent to either hand in the background, so that if you hit the hotkey it automatically equips it to that hand, but presently it doesn't work that way. Which is a real svck for spell and shield players...

the hotkey system is absolute fail for DW. I can't wait for shift-modifier or weapon-set hotkeys to be implemented.

This is one of the many "fails" of the Skyrim design, the UI being one of the worst I have ever experienced on PC. The only thing I can think of in your situation is if you are hitting the same hot key twice and not realizing it. Hitting a certain hotkey twice with magic assigned to it, will make it dual handed if you have the dual casting perk.
