I guess the third category of players, who have complained about Lydia jumping in front of them just when they release an arrow, or when they use chain lightning, must only exist in other players' games.

It is true that she will jump in front of you sometimes. Quite a few times i`ve had her leap right in front as I was about to strike and when you have a 2-handed sword in mid flow it`s hard to stop.
But there are plenty of ways to avoid hitting her. What I do:
1. Keep aware of the fact I have a Follower.
2. If I am about to fire an arrow, I never do it if Lydia is in the fight unless I KNOW I won`t hit her- And NEVER if she `s on her knees. That kills her.
3. If I make a strike with a sword and she dives in while I`m in mid flow, i`ve learned to turn away at that last moment so I swing and hit nothing. there have been the odd annoyance when I have a poison on my arrow and i`ve turned in actual firing mode (cos she jumped in the way) and wasted the arrow with its poison, but at least Lydia lives.
4. I also have using a Shout and avoiding Lydia down to a fine art. I can use a Fus Ro dah Shout while she`s fighting someone and blow away the enemy without touching Lydia.... Most times.
I know a lot of people want a perfect AI that`s almost telepathic, but failing that we, the Player, must make up for their limitations.
However, the Devs could`ve done a LOT more to make Followers much less `stupid.`
