- more radiant quests, so we don't encounter same adventures over and over again. Kill a dragon, bandit or giant become quite boring after a while
- when main story lines are finished, we are getting repeatable quests from the guilds, like check magic anomalies or kill a predator. It would be great if each guild received 3-4 hand crafted quests that can trigger some time later, after the main story line, but not necessarily instantly (you could for example hear Sithis calling or courier cound find you, asking for a swift return to companions). It would add a lot of life to the game and give you a feeling that you really are a guild leader. Also, increasing the number of repeatable quests would make more diverse experience.
- more "events" found on the road between towns or in the wilds. I really liked watching the fight between fire and cold mages, but there are so few such events. I don't say "add many", the lands needs to feel deserted, but about 10 new random encounters over 50 game hours would be great.
- new types of dungeons, with different feel, layout and textures
- new weapon/armour properties. Maybe make existing uniques feel more unique by adding some rare properties to them (black armour that deal damage to everyone around you is a great example)?
- maybe some more cursed items
- optional hardcoe mode - 1 life (you can do it yourself but interface is against you PLUS you will never feel the same level of danger compared to real hard core mode)
- improved magic system (I see a lot of people feel the same)
- some more wild animals scattered all over the map (boars, badgers, buffalos, snakes, more birds, skunks with poison cloud would be hilarious)
- seasons change (I saw this on skyrim jam videos and I LOVE it), maybe more weather effects like heavy winds
- add "cold" factor, you can't go naked to north areas or you will freeze to death. You need to keep warm (clothes, fire or magic) or you will die / suffer stamina penalty
