I found it in Apocrypha - don't remember which book I read to get there, pretty sure it was the main quest one.

It wasn't at the end. It was right by the first gate you had to open. Need one of the black pits with the tentacles.

I found one in each Apocraphya realm. I think. It's pretty frequent.
