Autosave and any follower(s) you might have with you is the cause of all your glitch woes in Dragonborn. Took me a while to figure that out.

It is supposed to go away automatically after the dialogue runs its course.
You did listen to the dialogue didnt you, cause I dont know what happens if you run around and wait randomly for hours.
If all else fails, reload a previous save.
And indeed turn all autosaves off. Should help with general performance as well.

Funny thing is, the tentacles coming out of the PCs eyes reminds me of the look most NPCs in Oblivion would give you in every conversation. Ever.

Its a random research quest.
Just ask Neloth if you can help with his research and eventually it will pop up.

Its one of his quests that pop up if you ask if you can help him with his research.
The one you get is random, but not radiant though there is a repeatable radiant one in there.
