It's fake, and I doubt Obsidian is putting a picture of the old placeholder super-mutants on the back.

1 disabled coments (so people cant post why its fake) 2 no shot of the disk itself (gee I wonder why, guess they dont have a lablemaker for the disk)and 3 you can pick up the boxes at some of the retailers already(some of them will give you the display box if you preorder in the store)

The fact that she doesn't show the disk is certainly telling. Stores sometimes get placeholder boxes to help advertise pre-orders and it's possible that's what she got her hands on - it's also possible that she made this herself. Either way I wouldn't go to the link she suggests and I'll bring this to the attention of the admin.
