So after getting banned from the Ultra-Luxe, Gamorrah and The Tops by winning too much money my achievement will not unlock. I have double checked at each casino to make sure I was banned, I've cleared my system cache and still nothing. Got any advice on what I could do next?

Does this achievement perhaps include the Casino in Primm and the one in Freeside as well? It's probably bugged, it wouldn't surprise me. Just wondering if this might be the issue instead of a bug.

All you need are the Ultra, Gamorrah, The Tops, and the Atomic out in Freeside. I didn't have any issues with the achievement.

That's what I figured. Primm didn't really make sense, but the Wrangler did.
