Ever since the update, my character has had an inexplicable Samoan arm tattoo, and strangely enough, when the tattoo appeared, the option to remove it disappeared. What happened?

I have the same problem. As well, my accent has changed. :S

Well, not sure how the tattoo applied in the first place. But when you choose a tattoo/scar, like voice and archtypes, they are permanent once you leave customization.

Along with the voice changing bug, I feel they should change how the 'permanent' customizations should be changeable. Seeing as you can often end up with an accent you definitely didn't choose, or tattoos/scars you later find don't look good, or as you stated, somehow ended up with them.

The tattoo being unremovable is a game mechanic, but the accent change bug definitely isn't. And It's REALLY annoying.
