Does it change anything? Do you get treated as one of the commanders? Does anything happen?

The only thing I noticed is that other soldiers sometimes adress you as legate.

no....imperial soldiers would just say: "legate" and move on, and stormcloak supporters will just call you milk drinker

You get a fancy nickname and that's it.

I think SC supporters call you milk drinker no matter your rank

No. The whole ranking system for both SC and IL was trivial.

If you join the SC you get General armor so you could rp yourself as a SC general.

While at other times they'll tell you to get out of the way, citizen.

Yeah... being Legate was kinda a let down. No additional quests. no invites to important meetings. no garrison with guards. Come on at least give me the fort I went to and cleared single handedly to show I was good enough to be an imperial.

It would have been really cool to get some Imperial soldiers for companions as well when you were the Legate. But...
