Hi - going to be vague to avoid spoilers, hope this makes sense:

In one of the first areas I got a quest to kill bandits at a moon camp (forget the exact name). Went there and cleared the bandits.

Then when I'm going through the companions questline doing the tasks, one of them is to kill the bandits at the same moon camp.


As soon as I picked up the quest, it immediately went to the step to talk to the questgiver. When I fast travel to the camp everything respawned so I got double of the loot.
But when I talk to him there was never any real option to turn in the quest.

It did eventually allow me to progress in the storyline, AFTER i went and recleared the camp even though i was on step "speak to dude" so it isn't game halting, but that quest is now greyed out in my journal with a hollow diamond - i can't finish it? Odd for having cleared the camp 2x.

What does the hollow diamond even means ?

That it wasn't finished.
