I finished my second play through a few days ago, and the game is lovely. However, my second play through, I decided to get "Mostly Flesh and Steel," "Clean Hands," "Ghost," and "Shadow." It was actually easier than I thought it would be- though in all fairness I was playing on normal.

My problem is that upon completing all levels with no kills, no detections, no killing main targets, and skipping Granny Rags in the Flooded District, I was not awarded Clean Hands, Ghost or Shadow. I did get Mostly Flesh and Steel, however.

All missions in the mission select screen show no kills or detections. Now, normally I'd assume this was user error and go back and do it all over again, but numerous people are reporting this problem.

Any idea what's wrong, Bethesda/ Arkane? And, if so, how do I fix it? Thanks.
