Found a bug, not sure if it's new, but I know it obviously hasn't been fixed. Reloading previous save fixes this, however this bug is something that should be prevented anyway.

Problem: Placing Space Helmet in Boone's inventory equips the Space Helmet and deletes the Beret permanently.

Steps with how this happened for me:

Location it happened: Broc Flower Cave, in the lab area (Might help?)

-Boone is wearing his Beret (Which he never wants off his head)
-I open equipment through companion menu with Boone (Companion)
-I trade him Space Helmet (To give me some free space)
-Noise of equip happens
-I close menu after trade complete
-He is wearing the Space Helmet instead of his Beret (As it would happen)
-He then asks for his Beret back
-His own Beret has disappeared. It is not dumped on the ground, it is not in his inventory, it is not in my backpack. It has vanished from existance.

So after you reloaded the game does the beret still disappear when giving boone the space helmet or did it just glitch that one time. What happens if you give boone the beret he gave you?.. somewhere along the line. When did he give me a beret? I forgot.. I've been wearing it the entire game due to my characters poor shooting skills. Little red beret, reinforced leather armor and authority glasses.. Yes, my character is the ultimate porm star of the wastes.

Maybe that's what happened?

After you re-load, if you do it again, it will happen. If you give Boone the beret he gave you, it will happen. And he gives you the Beret once you've fulfilled his requirements for him to join you.
