(1) I cannot remember the quest name, as the part of the glitch I am reporting about is that the quest won't show up on my quest log. I am currently on the part of the Companions Questline where you return from getting revenge on the Silver Hand, and you find that they attacked Jorrvaskr and killed Kodlak Whitemane.
Issues:

-When I was given the quest to go to Driftshade Refuge the quest did not show up in my log.
-Once I got there, Vilkas was not there. But I proceeded nonetheless, cleared the refuge. Then fast travelled back to Whiterun, find some dead Silver Hand, Aela and some other companion are standing there with weapons drawn. No sign of Vilkas, and no quest in my log to "track" to see where he might be to turn in my quest.
-Companions questline is now at a complete standstill.

(2) Also when I was doing the Dragonborne questline, I was sent to find Urag in The College of Winterhold to ask him where I might find the Elder Scroll.

-When I talked to him there was NO dialogue option to ask him about the Elder Scroll, or anything outside of the dialogue that would normally be available to you at any other point in time. I tried restarting the xbox, and still, nothing. (I eventually just watched youtube videos until I finally found the damn scroll, but fixing this bug would most likely help a few people out, if it is applicable to more people than just me...).
