I think before you get a house she waits for you at Dragonsreach (Whiterun castle). Check there.

what dragonsreach?

The castle in Whiterun. Assuming we're talking about the same companion.

what dragonsreach?

The place you -got- her, if you're talking about Lydia.
You should probably read the zones you enter.

If it's Lydia, she can die. I was playing a little this morning before I had to go in to work. My orc warrior/smith is level 16, and I had Lydia and Barbas (from the Clavicus Vile quest) following me around because I was working on that quest. I found what looked like Bleak Falls Barrow on top of a mountain in the far northwest coast, so I tried to get to it. Eventually I did, found a word wall on top of the hill, but when I approached it a dragon priest appeared out of the sarcophagus and the fight was on. I got killed about 3 different times. Eventually I equipped a resist frost necklace I had in inventory, downed a resist frost potion, used Berserk as soon as he appeared (double damage inflicted and take half damage for 60 seconds) and switched to the Become Ethereal shout to buy time to heal if needed. Both times I managed to win the fight, and both times Lydia died. I wound up reloading and leaving it for a later time.

The Sven guy vanished on me.

I gave him a sword which he equipped for a while. Then he suddenly had a bow and arrow, even though it wasn't in his inventory! Then he vanished. Into thin air. Lol. I re-loaded and eventually sent him back.

i think its just difficult for them to die because they just yeild when critically injured which stops the enemies attack on them

It'll depend upon circumstances. The game won't let you have more than one human companion at a time, and when a quest forces you to take another it'll dismiss your first. If dismissed they go home. Home for Lydia is either at Dragonreach in Whiterun, or your house (if you own one). I found her in my bedroom when I went looking.
