I'm having similar issues.
Whether I shoot manually, or with VATS, I am unable to hit the target. Using a missile launcher does damage, though not because I hit the target; it's only residual from the blast. It's even worse when using VATS: targeting the head causes the view to momentarily dip down offscreen, and then it bobs back up. Engaging VATS at any body part, still results in a miss, even with a 95+ percentage to hit. In one expirement I was crouched directly behind the enemy in the open and I still missed, repeatedly.
Even the enemy cannot hit me with their own firearms or projectile weapons. A blowfly spit at me for at least five minutes, and even though I was standing directly in front of it none of its projectiles would hit me.
Melee weapons work as normal, although striking a target causes them to rebound away from me as if they're in zero gravity, then I have to race after the corpse to try and loot it.
Sometimes I am unable to activate doors or gates (those I have even used previously), and I phase through the gates as though I were a ghost.
Attempting to talk with "Dogmeat" is also glitched. If I get the four options to come up, they repeatedly flicker so I have to try time it to activate them. And forget trying to get him to respond to any commands: If I click on him to 'go' somewhere, I'm unable to then make him 'stay', so I just end up canceling the action.
This is really starting to aggravate me and I have not found (so far) any means to see if there is an update or downloadable patch to fix this crap.
(Only as an FYI, I am currently level 37, so I do have a pretty good grasp of the gameplay and what is supposed to be normal, which is why I know these things are major glitches. I'm contemplating restarting, but I would not want to have a repeat of this same crap either). I"m hoping we get some answers from the gamesas crew, or whether there is a patch or download to fix these issues.
(second edit): I found a VERY temporary fix by returning to the Corvega plant, and going up the stairs above the main door and 'phasing' through it. I fall through the doorway, and when I land on the ground beneath the floor and turn around I find myself facing the lower door to the plant, which is clickable from this side and so I enter the plant. Inside Corvega all firearms (mine and the enemy's) work properly. Howevever, when I leave the plant, the same problem with firearms not working will happen shortly thereafter. I dismissed Dogmeat to Sanctuary, as I'm not sure if he was part of the original problem, though with or without him I seem to experience the firearm bug; I was annoyed with Dogmeat getting in the way most of the time anyway, so I generally use him as a pack mule for carrying extra gear.
Somehow the game fixed the issue. The problem always seemed to start when I chose to use VATS while outdoors (although it worked fine while indoors, when the game would have to load an interior map), so I stopped using it for quite awhile; instead, I loaded a save game and then I would manually shoot at enemies, never using VATS. Now, while this was not a perfect solutions (for obvious reasons: not getting the advantage of strategically selecting targets nor scanning for targets), at least I could still shoot at targets, not to mention that gates and doors would work properly again, as well as my defensive turrets being able to hit targets. After several hours of playing like this...manually shooting targets and saving my progress...I decided to try VATS again, and i found that the problem appeared to be fixed and it has been working properly since. I've tried to figure out what actually caused the problem to begin with, and I have a few ideas (although they may have absolutely nothing to do with it): first, do not save your progress while swimming or while under water, and do not quit the game without first exiting to the main menu (as I'm thinking that may have corrupted the files). For those still experiencing the same problems outlined in the earlier posts, I would recommend that you change your difficulty setting to "very easy", before you travel to protect any settlements which you are notified as being under attack, as this allows you to fight the enemy with melee much easier. After the battle you can always change the difficulty setting back to what it had been previously, but at least this method will give you an easier time of it while being forced to defend your settlements with only melee.
