Unfortunately the mod author has abandoned the mod. It had a "flat" file but this was removed for some reason. This is his last post from the Nexus.

Unfortunately , I finished the game and didn't like it enough for another playthrough , as a result I don't have the game or the modding tools installed anymore.
For the people who want the flat map and are willing to play without any markers , you can do it yourselves with NIFskope :
1. download nifskope from here : http://sourceforge.net/projects/niftools/files/latest/download?source=files
2. open "/Data/Meshes/PaperMapHeight/mesh.nif" in nifskope.
3. right click in the viewport , on the actual terrain object.
4. select "Transform >" / "Scale Vertices"
5. change Z to 0.0 (leave X,Y unchanged at 1.0)
6. click "Scale"
7. Save
It actually is _that_ simple , don't change anything else.

I am am thinking a flat paper version that would incorporate one of the in-game maps. Preferably the one that has the fold marks on the page.
It was also suggested, in the Nexus thread, to implement something on the sides to cover the blue void. This could be a good place for your hands, although it may be hard to set them to your race, gender and skin tone or current gauntlets.
Furthermore, there seems to be an issue with it displaying the map markers properly. I could live without that as I like the "Better Quest Objectives" by - whickus. I am not sure if that mod covers all the quest in the game yet, but still.
This would also prevent any fast travel, but I am ok with that as well. I would like some sort of multiple custom map marking ability. I could see it as being marks in red, like notes in blood or just black from an "x" or " + " or " - " made from charcoal.
I just don't posses the common knowledge and talent to pull something like this off.
I'd hoped that one of the talented modders of the community would take on the obviously welcomed task of doing something like this.
