Another one (similar, but different) was when I should demonstrate the whirlwind sprint. Lydia had decided to place herself right in front of me. So I bumped into her, Arngeir called "Come, Dragonborn! Attend to master Borri!" I ran in a circle to get Lydia behind me, then she did of course block my way again, the procedure repeated itself three or four times while Arngeir tried to make me "attend to master Borri". He must have thought I was an extremely unfocused student.

Now I always tell my follower to wait inside High Hrothgar.

Farkas sends me on a job to whack some Forsworn boss at Dragon Bridge Overlook. I have the drop on the boss, peering out over the ledge. I let fly, svcker-shouting him with Unrelenting Force, sending him over the edge. I kill off the other savages at the site... and I never get notification that he died from the fall. Turns out he fell into the Karth River and survived. I then had to go chasing him down some distance past the Dragon Bridge itself before I find him again.

And then my crossbow sneak attack shot wasn't lead in front of him enough.

Courier went all nudist on me. Acts like everything is normal but he ain't wearing a stitch apart from his loin cloth.
