I read this well over a decade ago so bear with me.

There was a plague that killed all the advlts. The story follows a female and her younger sibling as they try to survive by scaviging the neighbor hood. Some kids form gangs, I think the protagonist forms her own group of outcasts.

Reminds me a bit of a specific episode of Star Trek:OS. (Miri, I think it was)

Do you remember if it was sci-fi/fantasy/horror or more of an alternate "real world" setting, if you know what I mean?
