The non linear format Combined with the scale of the game will lead the underlying code into many different states, testing all of these states is merely impossible. One important aspect of this game is personalisation, right?
Taking this into account Bethesda need a process for us to (diplomatically) show the impact of the issue on the gaming experience and for them to triage this and assess how viable a fix can be within a certain patch development cycle given the numbers of users affected by it.
1.2 patches fix major graphics performance issues on both consoles that impact large numbers, I guess this was their focus for this development cycle.
Just remember, this is an excellent game and also remember this is a key title for Bethesda as a business, they want it to be a success as much as we do.
