You can use the Wrye Flash and in the Bashed Patch creation you can reduce the chance of dismemberment and limbs explosion.

Ask Arwen. She made one for Fallout 3, she'd probably be pretty happy to hear someone else wants no gore...On second thought, search FO3 Nexus for "Gore No More". I'm not sure how she made it, and it might work in NV.
