I noticed about 2 days ago my follower Lydia equips automatically an Imperial Helm in her head. After interacting with her and removing it from her inventory, she'll proceed to equip the best helmet in her inventory. But after a few minutes (or maybe some save/reload or transitions between maps), she'll have ANOTHER Imperial Helm in her head, ignoring the best helm in her inventory. I repeated this about 5 times and ended with 5 Imperial Helms in my inventory and Lydia remaining the same.

Tried to kill & resurrect her through console commands without avail. Also sent her to home and talked again to be a follower, removed all her inventory... everything without avail.

Any ideas? My list of mods is this:
