Messing with Vanilla?

So does this DLC add anything that can mess up the vanilla game?

For example Dawnguard, where random attacks in town were killing my favorite NPC's and needed merchants.

Not that i know of -its another island/cell completely seperate apart from 2 quests

The group of 2-3 hostiles who launch the main quest can spawn near NPCs, but that's about it.

Like said above, probably just the cultists. I haven't seen anything like the Vampires.

Yeah, the cultists will attack you. Even if you initally go to Solstheim and don't finish the MQ but head back to Skyrim, the cultists will randomly find you.

Do those end after the DB MQ or will you keep seeing them (like the vamps and DG in DG)?

I think it ends after the DB MQ.
I would suggest traveling to a town you do not like 1st after you get DB as these cultists may kill bystanders, I lost a smith I like this way.

I just finished the MQ a week or so ago. I haven't encountered any cultists. Some DB side-quests have you travel to Skyrim and back for specific reasons. Still, no cultists.
