Okay , so I'm level 29 and been raiding dungeons with the same companion since level 2.
Now I lost him. This has happened once before, back then the game gave me a notification after a couple
of game-days saying that your companion has now returned to his home. So I fast-traveled back and
everything was great again. This time I'm not getting any notification. Have you guys noticed how long
it takes before the game gives you that notification ?

Alright, so I have encountered similar issues with my companion. What happens is this: When I am travelling on foot (yes I prefer it over fast travel), my companion will sometimes just stop for no reason, and stand around, regardless of commands that are given. The companion will acknowledge said commands, but will just stand around. I have found myself backtracking when I turn around to see no one behind me to find my companion on, what I like to call now, an official union break. If I am particularly distracted, and have covered alot of ground only to find my companion missing, I hit select, and wait in 1 hour increments (the game time, NOT real time) at my current location. The companion will usually turn up (pop) into my view between waits. When the companion is unresponsive and just stands there as if glued to the ground, I do a save and reset my console, followed up with clearing the cache (YES this has helped on ALL my Bethesda games like Oblivion, Fallout, etc.)
I hope that the twitchy companions will be patched or fine tuned or something, but it is as yet, only a minor bug in the system. I hope that you are able to be reunited with your companion, as they make dungeoneering so much easier!
